It depends on what question is asked.
What OUGHT to appear is Margaret McDonald late Stewart and formerly
Crocket.
If she is asked 'what was your maiden name? then she should answer
Crocket, which will appear/
If she is asked 'what was your name before you married Mr McDonald, then
she should say Stewart.
Only if she is intelligent, coherent, uncowed by the official and
totally sure what the form requires will she give the full answer.
And if there is anything dodgy about the transfer of her heart and
hand from Mr S to Mrs M, she may be inclined to fudge over the results
anyway.

I can only answer from direct personal experience: my mother's marriage
to my father was her second marriage, but my birth registration shows
her original surname.

Yes, it would show her maiden name, not her former husband's name.

In Scotland the register will normally show her name as "Margaret McDonald,
previously Stewart, M S Crockett". Even if not, it will tell you the date
and place of the marriage*, and the marriage certificate# will tell you who
her parents were.
